Randstad are recruiting for a Qualified Social Worker to join St Helens Council's Integrated Adults Team as a Senior Practitioner. This is a full time role offering a great pay rate of £30+ per hour, DOE.

Responsibilities:
- Care Act Assessments
- Support plans
- Mental Capacity Assessments
- Best Interest Meetings
- Safeguarding Investigations
- Participation in the Duty rota
- Delivering and reviewing innovative services for our residents.
- Helping to reduce health inequalities, enhance life and support independent living.
- Work across friendly, dynamic teams alongside our district nurse colleagues with a focus on partnership working to achieve best practice and desired outcomes for our residents.
- Take pride in everything we do to ensure our services continue to be of an excellent and high quality standard.
- CHC, FNC and 117 funding/case loads.
- Support with CHC Assessments and recommendations, casework, MCA, Best Interest, Duty, etc.
Requirements:
- Social Work Degree or equivalent
- Social Work England registered
- Enhanced DBS
