Job Description:
We are seeking a dedicated and compassionate Social Worker to join our Mental Health Team in Doncaster. The ideal candidate will be passionate about supporting individuals experiencing Mental Health challenges and will possess excellent communication and interpersonal skills. As a Social Worker, you will work collaboratively with multidisciplinary teams to assess, plan, and implement interventions to support the holistic well-being of service users.
Responsibilities:
- Conduct comprehensive assessments of service users' Mental Health needs, including risk assessments and formulation of care plans.
- Provide individualized support and interventions to service users, utilizing evidence-based practices and therapeutic techniques.
- Collaborate with other professionals, including psychiatrists, psychologists, nurses, and support workers, to ensure coordinated and holistic care.
- Advocate for the rights and preferences of service users, ensuring their voices are heard in the decision-making process.
- Facilitate access to community resources and support networks to enhance social inclusion and overall functioning.
- Monitor and evaluate the effectiveness of interventions, adjusting care plans as needed to promote positive outcomes.
- Maintain accurate and up-to-date documentation of service user interactions, assessments, and progress in accordance with organizational policies and regulatory standards.
- Provide crisis intervention and support during times of acute Mental Health crisis, working collaboratively with emergency services when necessary.
- Participate in team meetings, case conferences, and professional development activities to enhance clinical knowledge and skills.
- Stay informed about current trends, research, and best practices in the field of Mental Health social work, applying new knowledge to enhance service delivery.
Qualifications:
- Bachelor's or Master's degree in Social Work from an accredited institution.
- Registration with the Health and Care Professions Council (HCPC) as a qualified Social Worker.
- Previous experience working in Mental Health settings, preferably within a community-based or multidisciplinary team.
- Strong assessment, communication, and interpersonal skills.
- Ability to work effectively both independently and as part of a team.
- Knowledge of relevant legislation, policies, and procedures related to Mental Health care.
- Commitment to promoting diversity, equity, and inclusion in all aspects of service delivery.
