Would you like to be part of a team helping to relieve the pressures on the NHS by supporting those
leaving hospital?
This post involves providing a commissioned service as agreed with the NHS at the point of discharge for
individuals living in their own homes. It will involve a variety of different task which may include,
collection of patients from hospital, helping them to get to appointments, basic food preparation, light
cleaning, shopping and collecting prescriptions for them. Successful individuals will be compassionate,
organised and highly motivated to provide high levels of support to clients, ensuring that those initial
weeks post discharge pave the way to returning to living independently at home.
Previous experience in a similar role is desirable but full training will be given. It is essential that the
successful candidate has a clean driving licence and access to a vehicle for work purposes, and a
willingness to take clients in their own vehicle to appointments. You will also require basic business
insurance. A mileage allowance will be paid (45p per mile).

The post is 20 hours per week working Monday - Friday between 9am – 4.30 pm, on a rolling contract ,
based on the needs of the service.
The pay rate is £10.82 per hour. Holiday is 5.6 weeks per annum, rising by an additional day with each
year of service up to a maximum of 5. Occupational sick pay is available after successful completion of
the probationary period.
