Proactively encourage users to access and signpost to appropriate services which may include housing provision, social support services; to promote and facilitate the harm reduction model of treatment provision in all aspects of work, to enable alcohol/substance users to adopt safer practices, healthier lifestyles and well-being.
As well as provide advice and information to service users about the range of services on offer both within the statutory and non-statutory sector and assist service users in accessing these.
The main duties of the role include -
- To support the delivery of services in assisting service users to maintain their recovery.
- Support service users in gaining understanding and insight into their own recovery, maximising engagement and retention and will act as a role model for recovery.
- To Provide and receive routine information requiring tact and persuasive skills
- To develop a relationship with service users based on respect, companionship and friendship within appropriate and transparent boundaries
- To promote all methods of communication and be able to effectively communicate with service users and their carers
- To promote social inclusion and develop and maintain community links whilst promoting equal opportunities
- To take into account any communication difficulties facing the patient. To demonstrate a safe level of inter-personal skills when managing individuals with complex needs
- To be able to accurately document care given and information received in service users clinical notes
- Deliver presentations to colleagues and group work to patientsTo contribute to clinical meetings, including weekly Multi-disciplinary meetings
