Key Responsibilities CLINICAL LEADERSHIP The Registered Nurse will maintain a nursing culture which is open to change, new ideas, concepts and innovations. This will be achieved by: Participating in the development and delivery of a philosophy of care which involves/respects the rights of patients carers and staff. The ability to communicate and negotiate with patients and their visitors in order to promote their comfort, safety and wellbeing. To be aware of barriers to effective communication which may be evident such as cognitive, expressive and receptive impairment, embarrassment, anxiety, pain, fear, visual/hearing, impairment and to seek guidance from senior staff if required to implement strategies.
Ability to relate to all staff and patients in a confident manner, demonstrating good listening skills and able to work effectively as part of team. To constructively challenge multidisciplinary practices which impact on care, providing drive and enthusiasm for effective planning regarding patient flow. To ensure patients are involved in decisions about their care and treatment and that they have sufficient information to do this. Creating, maintaining and enhancing effective working relationships.
Recognising areas of conflict and assisting in the management of this to promote a healthy working environment. Maintaining an awareness of national and regional initiatives. Participating in the individual performance review process. Participating in the implementation and monitoring of the Trusts Nursing Charter standards.
Acting as a positive ambassador for the Trust. Contributing to the Trusts Shared Leadership process. Striving for continuous improvements in the delivery of high quality patient focused care. Submitting new ideas/Innovations aimed at improving patient care.
To assist in reviewing referrals and prioritising according to clinical need. To engage patients in the consent process to facilitate compliance with their treatment programme. To accept clinical responsibility for identified caseload, prioritising accordingly and managing own time. To ensure that accurate nursing records are maintained for each patient.
To provide accurate statistical information in line with local standards and requirements. To be a visible, accessible and assertive resource for staff, patients and relatives providing support and guidance. LEARNING ENVIRONMENT/EVIDENCE BASED NURSING The Registered Nurse will support and contribute to a ward culture which is conducive to learning incorporating high quality evidence based practice into patient centred care. This will be achieved by: Utilising and contributing to resources within the ward environment which encourage evidence based nursing.
Promoting patient/carer independence through relevant teaching and support. Advise on the promotion of health and prevention of illness, teaching patients and their relatives where appropriate. Ensure patient and user involvement and feedback is instrumental in improving practice through appropriate education and training over a wide range of sites and disciplines both within the Trust and public arenas to raise awareness. Supporting and enabling professional development of team members.
Undertaking own personal development to meet identified needs. Contributing to the mentorship of colleagues as directed by the ward sister. Ensuring statutory training is undertaken. COMPETENT PRACTICE The Registered Nurse will combine theory and experience to demonstrate clinical competence in practice.
This will be achieved by: Providing expert tailored treatment, advice, consultation, support and guidance to patients, carers within referring trusts. Provide comprehensive pre-assessment information and prioritise patient admission from referring hospitals/community settings to optimise bed occupancy. Support, enable and assist the patient with a spinal cord injury to actively participate in their rehabilitation programme created by the multidisciplinary and encourage and promote physical or verbal independence in all aspects of their care. Delivering direct care to patients in accordance with National and Trust policies and NMC Guidelines.
To participate in multi-disciplinary ward meetings and case conferences in order to proactively identify appropriate referrals and support. To provide specialist assessment for patients with complex health needs. Continually assessing and evaluating the patients condition and the effectiveness of the care delivered with senior members of the team and ensures plans of care are updated as appropriate. Assist patients and carers with appropriate educational programmes, to include, but not limited to, bowel, bladder and skin management, assess effectiveness and understanding of training and adjust plans as required.
Promoting a multi professional approach to maximise the patients health and independence utilising resources appropriately. Maintaining up to date knowledge of current nursing issues/practices. Following appropriate training and assessment, use prescribed rehabilitation equipment within the department at the direction of the MDT. Able to demonstrate knowledge and skills related to airway management including, but not limited to, assisted coughing, tracheal suctioning cough assist.
Is able to break bad news to patients and relatives in a professional and sympathetic manner. To motivate and encourage patients, providing advice and equipment as appropriate. Participate in case conferences, MDT meetings and reviews as appropriate. To maintain and promote department/multidisciplinary communications.
To independently manage nurse-led and MDT clinics, making autonomous decisions and providing clinical interventions and advice. To develop self-care management programmes. To maintain own professional development within the statutory requirements of the NMC to include, maintaining and improving knowledge and expertise in current developments, nursing management and practice, attending courses, study days, etc. To resolve minor concerns and complaints from patients and visitors and pass serious complaints on as per policy.
There may also be a requirement to undertake other similar duties as part of this post in order to provide a quality service. These will be consistent with the level of responsibilities outlined above.
