Rehab and Reablement Assistant

If you are passionate about caring for people and want to help them to remain safe, well and independent in their own homes then this is the job for you!

We are looking for enthusiastic and motivated individuals who work well both individually and as part of a large team. Urgent Response is an Integrated Health and Social care Service offering short term Rehab & Reablement support to adults in their own homes across the City of Southampton.

Assisting individuals to maximise their independence with daily living tasks. The role will involve travelling across Southampton to support service users in their homes.

Some keys areas of the rehab and Reablement Assistant role include:

  • Supporting people to reach Reablement goals
  • Supporting with personal care, medication, diet, and fluids
  • Be proactive in spotting signs of deterioration to avoid hospital admissions
  • Keeping accurate notes
  • Be a good team player and enjoy working with people
  • Commitment and reliability is essential for this role.
  • Must hold a UK driving licence and have access to a car.

Various hours and rotas available which will include some weekend and evening working.

Basic Annual Salary is £26,609(FTE) plus either a 10% or 15% shift allowance

(10% will equate to £29,270FTE and 15% will equate to £30,600FTE)

Starting salary is £13.79 per hour + 10% or 15% shift allowance depending on working hours

(15% will equate to £15.86 per hour and 10% will equate to £15.17 per hour)

Plus £40 per month contractual car user payment and 45p per mile.
