Job description

The post-holder will be responsible for the management of a defined caseload kidney transplant patients and will provide specialist nursing support, advice, education and/or care to this specific group. They will carry continuing responsibility for the assessment of needs, the development, implementation and evaluation of programmes of care and the setting of standards within the Renal Transplant Department. This will include: Supporting the repatriation of newly transplanted patients. Providing transitional care support for young adult transplant patients in collaboration with the paediatric transplant team in Leeds. Providing patient and carers with support, advice and education, including participation in a transplant patient forum and the appropriate use of social media. Supporting the renal anaemia programme, including a drop in clinic. Co-ordinating and monitoring of blood test results, including clear communication with the Transplant Immunology and Virology laboratories in Leeds. Overseeing of post-transplant clinics. Involvement and delivery of transplant anniversary reviews. Working in conjunction with all relevant professional groups, including the advanced kidney care team, renal dietitians, renal pharmacists, renal psychologists, a cultural and health improvement officer, diabetes specialist nurses, allied specialties including radiology and urology, and primary care teams (using the SystmOne Renal Unit Hub). Support for transplant audit, quality improvement and research. Clinical Quality and Safety Carries continuous responsibility for a defined caseload kidney transplant patients including setting standards, assessment of needs and the development, implementation and evaluation of programmes of care following (clinical) protocols within Renal Transplant. Organises, prioritises and evaluates workload based on accepted referral and discharge criteria in line with operational protocols. Responsible for ensuring own practice is evidence-based. Assesses, plans, implements and evaluates specialist care or treatment regimes for patients requiring Renal Transplant, taking into consideration the lifestyle, gender, cultural background and preferences of the individual, making reasonable adjustments to care where this is appropriate. Works as part of the MDT fostering effective professional relationships to ensure effective communication and a seamless service for a defined caseload (patients/service-users). Works across organisational and professional boundaries as an expert practitioner as part of the multi-disciplinary team e.g. liaising with colleagues in primary care (District Nurses, GPs etc.). Promotes health education and health promotion with patients and carers, ensuring that they involved in decision making. Utilises interpersonal and communication skills in order to convey complex and sensitive, and potentially contentious information with empathy and reassurance, overcoming barriers to understanding where necessary. Provides appropriate written information to support verbal advice. Communicates effectively with hospital medical staff, general practitioners and nursing/midwifery staff giving specialist support and advice in the care and treatment of patients/service-users within their specialty and provide expert professional advice for other members of the healthcare team and patients as required. Utilises excellent communication skills to support patients/carers when breaking bad news and discussing treatment plans or withdrawing treatment. Demonstrates knowledge and understanding of the different roles in the multi-disciplinary team and liaises with and refers patients/service-users/staff directly to other professionals and agencies where appropriate. Practices within the Trust policies and procedures, proposing changes to policies within own area of practice where appropriate. Support would be provided to complete the non-Medical prescribing course once established in the role. Works within national policy frameworks and guidelines relevant to the patient group and specialty area, i.e. NICE, Mental Health Act etc. Adheres to Trust Health and Safety, Risk Management and Infection Control policy and practices to maintain a safe working environment and minimise risk to patients/service-users, self and other staff at all times. The post-holder will be required to provide interventions to promote self-care to minimise hospital admissions where possible. Reviews, interprets and acts upon both laboratory and point of care patient test results which may affect change to individual patient/service-user care plans. Liaise with senior staff as necessary. The post-holder is responsible for clinical data entry which generates information for use with other healthcare professionals. E.g. SystmOne and other specific databases relevant to the designated service. The post-holder will support the implementation of local / national standards in conjunction with relevant members of the multi-disciplinary team. Arranges admission of patients/service-users to hospital when required. Undertakes nurse-led clinics in the hospital and wider community if necessary as appropriate to the needs of the service. Monthly clinic run at Skipton General Hospital. To supply and / or administer medications according to Patient Group Directions in accordance with BTHFT Policy for the Development and Review of Patient Group Directions for the Supply and Administration of Medicines. Service Development / Improvement Undertakes regular audits using research and improvement methods in order to develop and agree action plans to implement changes in practice, and thereby improve service/care delivery. Disseminates audit/research results within the specialty Clinical Governance structure. Works in conjunction with other managers and specialist colleagues under the clinical leadership of the Medical Consultants in the specialty to develop and enhance the clinical service. Works in conjunction with the senior management team to deliver services according to the agreed job plan. Contributes to the development and review of Trust policies and protocols in relation to specialty area of care where appropriate. Actively contributes to the strategic management of the speciality supporting the development of the service, patient information, care pathways and user involvement. Actively participates in all aspects of the specialty and directorate clinical governance arrangements. Management and Leadership Has clinical autonomy over the management of the needs of a defined caseload. Participates in the investigation and response to formal and informal complaints and incidents. Acts as a role model and when required assist members of the team with planning, priority setting and decision making. Contributes to the achievement of departmental performance metrics. Exercises a personal duty of care in relation to equipment, resource and stock control utilising equipment and supplies appropriately. Responsible for provision and maintenance of accurate department based information systems including patient based processes, record keeping, care documentation and personnel data, in line with the Data Protection Act Education and Development Liaises with Ward/Department Managers to support and develop ward nurses and competencies in providing holistic care for patients/service-users within their specialist area. Provides education programmes and clinical support for ward/department, nurses/midwives and patients/service-users. Educates, individually or in groups, including the patient/service-user and their family/carers to understand the nature and management of their needs. Participates in a system of clinical supervision. Works within the NMC Code, ensuring continuation of professional development relevant to the role, including mandatory training, through revalidation requirements, and highlighting needs through the annual appraisal review process. Provides mentorship to both pre and post registration students where necessary. Acts as a specialist resource for defined area of expertise and provides a specialist learning environment for other health care professionals as appropriate. Supports and participates in the development and delivery of healthcare professional education programmes within the locale and more widely as appropriate. Research Has an understanding of relevant (clinical) research evidence affecting patients/service-users/staff within own caseload. Is aware of specialty-related research to enable the implementation of good clinical practice and other relevant research publications/evidence. Responsible for ensuring own practice is evidence-based. Be able to undertake relevant literature searches pertinent to patient/service-user care. Shares good practice, through publication, presentation and other forums such as journal clubs both within and outside the Trust. Contributes to the R&D agenda and initiatives within their department in collaboration with other members of the multi-disciplinary team. Maintenance of relevant specialty databases to facilitate audit and research in line with Information Governance policy. Participates in research projects as required, and works with the multi-disciplinary team to raise awareness of research within the speciality/department to patients/service-users and staff.
