Resettlement Mentor

ABOUT THE ROLE
As a Resettlement Mentor, you will contribute to ensuring supportive and positive transitions for our residents and participants, supporting them to gain and maintain secure accommodation, and support with move on plans for greater, independent living. You will provide pathways, support, and appropriate interventions to enable successful resettlements. 

You will be based across various locations in RBKC (Royal Borough of Kensington and Chelsea) and will work on a 7 day rota which will include late evenings.

RBKC services  specializing in supporting people with multiple needs, accommodation and community-based support. We help people that struggle with daily living, mental illness, substance and alcohol abuses, homelessness or offending backgrounds by providing them with support to overcome difficulties existing because of their life circumstances.

ABOUT YOU

We’re looking for someone with a genuine passion and felt purpose to help people, challenge stigma, and make a real difference to people’s lives!
We are looking for a team player, driven to provide high quality care and support to others, an effective communicator who is able to build rapport with others from various backgrounds. You will be compassionate, supportive, and empowering to others, whilst able to form effective, positive and motivational relationships.

OVERVIEW OF KEY RESPONSIBILITIES

• Lead on the assessment of referrals into service, ensuring a clear and supportive service.
• Support with the coordination of referrals.
• Develop appropriate move on plans, with a key focus on empowering them to achieve greater choice and control.
• Ensure R&Ps  understand their rights and responsibilities, with access to the right tools and resources to support them to gain and maintain secure accommodation.
• Develop effective internal and external relationships.
• Provide advice, guidance, and support, including life skills training.
• Recognise signs of deteriorating mental health and initiate appropriate interventions to prevent a crisis.
• You may need to attend appointments alongside R&Ps when required.
• Admin duties will vary, including using our Salesforce Inform system to update records ongoing.

Other responsibilities than those described above may be required to be undertaken from time to time and will be expected to be performed to as long as it is within the capability and level of the position. 

KEY CRITERIA

What we are looking for:

• IT Proficiency, ability to learn new software programs, basic Microsoft experience is required
• Experience of working with people of complex backgrounds, including in the criminal justice field, mental health, and/or substance misuse or a good understanding of the sector
• Understanding of the housing and social needs of people with multiple and complex needs
• Able to influence and negotiate positive outcomes with others

What we would like, but not essential:

• Appropriate qualification: NVQ/Diploma Level 2 in Health & Social Care/Community Justice, or professional equivalent
• Experience of providing housing support and a practical assistance within a residential or outreach support role
• Experience creating co-produced support plans and providing appropriate interventions for service user and liaising with other professionals
