Key Highlights:
- Complete Care Act assessments, support plans and reviews of package of care and care home placements.
- Navigate cases in accordance with Care Act, Mental Capacity Act, and Human Rights Act.
- Build meaningful connections with individuals, families, and the community.
- To work on duty to screen new referrals and respond to telephone calls.
Necessary Requirements (Your application will not be considered if you lack all three):
- Degree in social work
- Up to date Social Work England registration
- Significant front line experience in adult services within the UK
