The Heart Failure Specialist Nurse will work with the wider Heart Failure team to ensure that patients within the population we serve have timely, equitable access to high quality, evidence based Heart Failure care throughout their entire pathway The post holder will Promote excellence in nursing and medical care of Heart Failure patients Work closely with medical teams (non-cardiac specialists) and advise on current guideline directed medical therapy for patients with confirmed Heart Failure Present and discuss patients at MDTs Deliver patient centered education and advice regarding diagnosis and management to the patient, relatives and carers to include promotion of appropriate self management strategies Work in partnership with other specialist in other services e.g. renal/diabetes/respiratory who are involved in managing patients with Heart Failure Take a lead in the discussions regarding palliative care and end of life care for patients with Heart Failure. Will be responsible for contributing to the ongoing development of clinical practice, teaching, research, clinical audits and standards of care within the service, including development of policies, protocols and guidelines and contribute to service developments in collaboration with multidisciplinary colleagues.
