Job description

Managerial and organisational effectiveness To maintain an up to date knowledge and be fully conversant of developments in local/national pharmacy and medicines management procedures. Be fully conversant on the usage and appropriate maintenance of all IT To maintain all records and documentation necessary to keep accurate contemporaneous notes. To maintain computer files for patients information and records ( For example RIO). Assist in the collection and analysis of workload data when Identify areas where the technical pharmacy service can be developed and plan changes in liaison with senior nurse colleagues. To advise patients and other healthcare professionals on the safe and effective use of medicines adjusting the level of advice as appropriate to overcome barriers to understanding (e.g. hard hearing) referring to the pharmacist where appropriate. To anticipate potential problems and communicate with line managers in a timely To liaise with colleagues and other agencies, including social services, to ensure continuity of care. To participate in clinical supervision with a registered pharmacist on a monthly To escalate any concerns regarding a patients care, health and wellbeing to a senior nurse Clinical services - Patient care To provide and assist with the health care of clients in their own homes and elsewhere in community. Participate in medicine reconciliation to improve medicines optimisation across the primary and secondary care interface. Participation in medication history screening, involving problem solving, communicating with patients, GPs, and other healthcare professionals. Participate in the development and expansion of role of pharmacy Participate in Whittington Health medicines management Participate in the extended roles for technicians as the opportunity arises and other clinical roles subject to availability of suitable training. Encourage adherence to current guidelines e.g. BNF, NICE and suggesting suitable alternatives if appropriate. To assist nursing staff in the delivery or administration of medicines when required with relevant advice to promote concordance and pharmaceutical care, in line with local policies and procedures. To assist in the development of Standard Operating Procedures and other medicines management tools. Role and Personal Development (teaching, training, and research) Self directs completion and prioritisation of own Enforces trust policy with respect to formulary; unlicensed medicines and departmental Standard Operating Policies. Adhere to trust policies/Health and Safety and COSHH Whittington Health performance appraisal system will be used to assist personal development and to improve the range and efficiency of the services within the department.
