Hywel Dda University Health Board are delighted to offer this exciting opportunity to join our Medicines Optimisation Pharmacy Team working in primary care. This post will provide opportunities to directly influence patient care through working in the GP Practices run by Hywel Dda University Health Board.
This post would be ideal for someone looking to develop expanding on their understanding of operations and processes within the Health Board and primary care, and demonstrating the impact of the role in practice. These roles will involve direct patient working alongside our clinical Pharmacists as a key member of the team, contributing to prescribing of the highest quality, safety and value, supporting Medicines Management and prescribing processes in the GP Practices.

You will work as part of the multidisciplinary practice team providing primary care services to patients and will actively develop the role of the Pharmacy Technician in GP practices. The successful applicant will have an ability to develop effective working relationships, to challenge yourself and others, and to prioritise your own work. You will need to be reliable, efficient and responsive with the ability to demonstrate a flexible approach to the needs of this challenging and growing service.
You will provide support to GPs, Pharmacists, other clinical staff and reception teams, co-ordinating discharge summaries and medication reviews, playing a central role in repeat prescribing processes, liaising with our patients, carers, hospital and community pharmacies, responding to medication queries and updating medical records.
You will have excellent communication skills with a demonstrated ability to work autonomously and collaboratively to ensure patients with complex pharmaceutical needs are reviewed effectively, minimising the need for GP intervention.
The post is based in and around Carmarthenshire, although there may be a requirement to travel Health Board wide if needed.
Hywel Dda University Health Board is the planner and provider of NHS healthcare services for people in Carmarthenshire, Ceredigion, Pembrokeshire and its bordering counties. Our 11,000 members of staff provide primary, community, in-hospital, mental health and learning disabilities services for around 384,000 people across a quarter of the landmass of Wales. 48 general practices, 47 dental practices, 96 community pharmacies, 44 general ophthalmic practices and 17 domiciliary only providers and 11 health centres;
Numerous locations providing mental health and learning disabilities services.
You should have a passion for improvement and patient-focused care and should be able to work well as part of a multidisciplinary team. You will be required to accurately maintain patient records and analyse data effectively. Excellent communication skills is a prerequisite.
