An exciting opportunity has arisen for a highly motivated Senior Practitioner to join our dynamic Hospital Social Work Team. The hospital team forms part of the new Early Intervention & Wellbeing Hub that has been developed following our recent review of Social Work.
The Hospital Social Work Team have a crucial role in providing support and assistance to adults and their families following admission into hospital. We work collaboratively with health professionals to undertaken assessments to facilitate smooth, safe, and timely transitions from hospital to home or alternative care settings. We support people in hospital during times of crisis, supporting people to successfully navigate their hospital journey. The team play a pivotal role in enhancing the overall wellbeing of individuals whilst they are in and discharged from hospital, ensuring they have the necessary support in place and promoting a person-centred approach within the healthcare system.
As a Senior Practitioner you will assume a leadership role, contributing to the overall effectiveness of the Social Work Team. Responsibilities will include:
providing guidance, support and leadership to the Social Work Team, ensuring effective collaboration and adherence to best practices.
Supervision of Social Workers providing guidance, support, and mentoring to enhance their professional development.
Holding a small caseload of complex cases which require advanced Social Work skills.
Development work and research that will contribute to the overall development of the team.
Performance management and quality assurance ensuring best practice, evidence-based decisions, adherence to legislation and BCBC processes.
Collaboration with health professionals and partner agencies.
Deputising in the absence of Team Manager.

We are seeking an enthusiastic and resourceful senior Social Worker who will have the ability to respond positively and creatively to the needs of the people we work with. Essential qualities include strong communication skills, enthusiasm, proficiency in multidisciplinary working, and the ability to assertively build and sustain relationships with key partners.
The ability to greet customers through the medium of Welsh is a requirement for this post.
Protecting children, young people or adults at risk is a core responsibility of all council employees.
Criminal records check by the Disclosure & Barring Service (DBS) is a requirement for this post.
