Senior Social Worker Hospital Team

Job Role: The Senior Social Worker (Adults) will provide advanced social work services to adults with complex needs. This role involves leading and managing complex cases, supervising junior staff, and contributing to the development and implementation of service improvements. The Senior Social Worker will work collaboratively with individuals, their families, and a multidisciplinary team to ensure the delivery of person-centred care.
 Key Responsibilities:
- Conduct comprehensive assessments of adults with complex needs to determine eligibility for social care services.
- Develop, implement, and review personalised care plans in collaboration with service users, their families, and other professionals. Manage a complex caseload, ensuring timely and effective interventions to safeguard and promote the well-being of adults.
- Provide expert advice and guidance to Social Workers and other professionals, supporting them in managing their caseloads and complex cases.
- Lead safeguarding investigations and ensure compliance with statutory responsibilities and organisational policies.
- Work closely with health, housing, and other partner agencies to deliver integrated services and support.
- Participate in and contribute to multidisciplinary team meetings, case conferences, and care planning sessions.
- Maintain accurate, up-to-date case records in accordance with legal and organisational standards.
- Identify and implement service improvements, contributing to the development of policies and procedures.
Sirona Medical will be providing the Social Worker on an agency contract. 
As a Social Worker, you must have a degree in Social Work, experience in woking in Hospital setting, Social Work England Registation.
