Community Integrated Care is currently seeking a dedicated, compassionate and experienced health and social care professional to join the team as a Service Leader based in Worcestershire.

As Service Leader in Worcestershire, you will be managing a team of Advanced Support Workers working across a brand new, purpose built block of bungalows for seven people as well as several new single person tenancies across the county.  This is a great opportunity to make a genuine difference where you will be responsible for leading an engaged and motivated team to change lives and deliver person-centred outcomes for every person we support. Our overall aim is to help people live their best lives possible.

You will take the lead by driving performance, promoting reflective practice, setting the direction within services, promoting a positive and passionate culture, and ensuring that services are effectively resourced. 

The people supported in Worcestershire will have a variety of support needs, including autism, complex learning disability, mental health needs (for example Emotionally Unstable Personality Disorder and schizophrenia), forensic backgrounds or a risk of offending.  Some of the individuals may present with varying levels of behaviours that challenge and in some cases risky behaviours. As Service Leader, you will have extensive experience supporting people with similar complex needs, particularly understanding deliberate self-harm and risk management. A background in trauma/psychologically informed care and/or an awareness of attachment theory would be preferable.

The ideal candidate will have previous experience of implementing Positive Behavioural Support strategies and coaching teams to reduce behaviours that challenge and the use of restrictive interventions. You will be calm and confident in times of crisis, supporting your teams through challenging circumstances. In addition to leading reflective practice and de-briefs with your teams, we also expect you to engage in supervision and take feedback. Experience of supporting people to transition from in-patient settings and working closely with multi-disciplinary teams and clinicians, would be desirable.

We’d like you to have experience of using structured professional judgement tools (e.g. HCR-20, SVR-20) and experience of case management.  Although this is not essential. You will be comfortable using basic IT systems that are required within the role - training can be offered to support this area. Candidates will hold a Level 4 in PBS, or be willing to undertake this within their probationary period, which the company will organise.

You will also hold a professional qualification in Health and Social Care, i.e. Nurse, Social Worker, SVQ Level 3 plus supervisory units (Scotland) / Care Management Level 3 (England).
