Social Care Assessor

This is an exciting opportunity for a Social Care Assessor to join our North Monmouthshire Integrated Team. We work in and around Abergavenny and the wider Monmouthshire area to support people with a range of impairments to live their lives. We work in local hospitals to support people who need help with their discharge planning. We welcome applications from staff who want to work full-time or part-time. These roles will form part of the integrated service and will work across the community and hospital settings. Successful candidates will be able to work autonomously, as well as part of a team.
