SalaryDisabled Children’s Service Targeted Support is looking for two Social Care Assessors.

As a Social Care Assessor in Disabled Children’s Service Targeted Support, you will play a crucial role in assessing the needs of children and their families playing a key role in identifying their needs and support which may promote their outcomes.
Your responsibilities will include:
- Conducting thorough assessments of children and families’ physical, emotional, and social needs.
- Developing personalized care plans to meet individual requirements which are co-designed with families and young people.
- Collaborating with children/ young people, families, and healthcare and education professionals to ensure the best possible outcomes.
- Monitoring and reviewing care plans to make necessary adjustments in order to promote a positive future.
- Providing support, guidance, and information to children/ young people and their families.
- Maintaining accurate records and reports.
Although understanding of disability and the challenges this brings for individuals and families is helpful, it is not essential. We will provide a robust induction and support.
