Job Role: Social Care Assessor
You will require excellent communication skills, both verbal and written. Good IT skills are essential. Successful applicants will be committed to working as part of a team, to providing quality services that promote principles of empowerment to support adults at risk, their carers and advocates to achieve their outcomes. You will be required to use assessment, judgement and decision-making skills with a focus on supporting the adult to manage risk and identify need for support and assistance. Knowledge of safeguarding procedures, current legislation and government initiatives will be beneficial to the role; also, a willingness to look forward and explore new and creative ways of working.
As a Social worker you must have a degree in Social work with a minimum 2 years post qualifying experience and passed your AYSE, experience in an NHS Establishment or a local authority is desirable.
To act in accordance with SWE registration requirements and Code of Conduct for exercising accountability and professional practice.
To provide professional Social work assessments to support the commissioning process in line with care management and continuing care.
Employers will be keen to see that you have a friendly and caring attitude, and understand and respect people who need healthcare support.
Previous experience and/or qualifications in Social work are essential to apply for this role.
