Job Description

We are seeking a Social Care Officer to join our fast paced and dynamic Hospital Social Work Team within the HSCP.
You will work closely with Health colleagues and the wider multi-disciplinary team to achieve the best outcomes for our service users.
The post is will be based between acute and community health sites, as well as the opportunity to do hybrid working.
