An interesting opportunity has arisen for a Social Care Practitioner to join the Community Hospital (CH) and Community Support Beds (CSB) Discharge Team primarily to arrange discharges from Community Hospitals and to help support the flow of Community Support Beds within DCC care homes throughout the county.
The working pattern will be negotiated on appointment, but it is anticipated that the 25.9 hour post will be worked across 4 or 5 days a week. Applications are particularly welcome from people who live in the Buxton or High Peak area as the role will focus on the Cavendish Community Hospital at Buxton and the Stamford Unit on the Tameside hospital site.
Although much of the work can be completed remotely, occasional visits will be required. You will also need to be flexible and willing to support discharges from any location throughout the county, to maintain cover during planned and unplanned absences of other team members.
The post holder will be a key player in supporting health and care systems during a period of increasing pressure on hospital beds and the wider system. Your office base will be agreed on appointment, to facilitate smarter travel, and mileage and travel time will be paid in accordance with the current travel policy.
Experience of assessment and reviewing and a knowledge of local resources will be an advantage but applicants with the ability to utilise transferable skills, enthusiasm, and a willingness to learn will be equally considered.
You will need a commitment to person centred practice and sustainability, taking a home first approach in line with the Best Lives initiative. You will need good organisational and time management skills to achieve timely, sustainable discharges and positive outcomes for the people who are being discharged from Community Hospitals or Community Support Beds.
You will also require excellent communication and inter-personal skills to both support the people and their families, whilst enabling you to liaise and maintain positive relationships with hospital and care home staff and workers from other disciplines.

The job may require unsocial hours working including weekends and Bank Holidays. Payment arrangements will be detailed in any contract offer made.
Saturday and Sunday hours will be paid at plain time where it is part of your normal working pattern unless it is between 7pm and 7am, where enhanced rates of pay apply. Enhanced pay rates are paid for Bank Holiday working.
