Temporary Social Care Practitioner

Hours per week: 35-37 Monday – Friday, normal working hours
This opening assignment is for 3 months

Enhanced DBS required for this role
The role of Social Care Practitioner will require collaborative working with Social Workers and other professionals and colleagues, delivering bespoke intervention work across the South Tyneside Borough.
Key Responsibilities
Use active listening skills, observation and communication to build relationships with adults, their families and informal network supports and provide advice and guidance to help support them to navigate their way through systems.
Safeguard individuals in a way that supports them in making choices and having control in how they choose to live their lives. Ensuring that the ‘Making Safeguarding Personal’ ethos is at the heart of all practice.
Work directly with the person to support them to undertake an assessment of their needs using our Let’s Talk Together approach and enabling them to identify the outcomes they wish to achieve, whilst maximising their independence and choice. This will involve drawing on the support of families and informal carers and adopting a strengths-based approach.
Understand the roles and accountabilities of other agencies and professionals in order to refer, liaise, consult and work effectively in order to meet identified needs.
Work with the person or carer to develop and agree a support plan to achieve the identified outcomes to meet assessed eligible need, within resource allocation.
Under supervision, ensure the proactive and appropriate positive management of risk with adults, their families and carers and report accurate information and observations to line management for their consideration and further instructions.
Keep appropriate, accurate and timely records that describe and support analysis of the adults’ experience, clearly identifying the adults voice and demonstrate any decision-making rationale, whilst maintaining a high level of data quality.
Prepare and present reports of a statutory and non-statutory nature.
Contribute to the reviews of support plans as required, ensuring the right level of support is delivered only for as long as is needed and at a reasonable cost, advising the person and their carers of alternative options as appropriate.
Ensure practice is informed by evidence, research and theory and meets with council guidelines, policies and procedures as well as local and national professional standards.

Qualifications:
A good standard of general education, including Maths and English
A relevant Level 2 or equivalent qualification in either Social Care, Health or Housing related field
Level 3 Diploma in Health and Social Care (Adults) or a willingness to undertake qualification within the first 12 months of commencement of role
Knowledge:
A good working knowledge and ability to use information technology and related systems
Knowledge of Social Care legislation and guidance
Experience:
Experience of working within a Social Care, health or housing environment with people with support needs. This may include people with a learning disability, autism, older people and people with physical and mental disabilities
Experience of providing information and advice while working in direct contact with people, families and carers in either a paid or voluntary capacity.
Skills & Abilities:
Effective decision-making skills which will involve considering risks
Ability to prepare and present reports of a statutory and non-statutory nature
Ability to use active listening, observation and communication skills to build relationships with adults, their families, carers and informal support networks
The ability to understand and analyse the needs of adults, their families, carers and informal support networks by gathering information through direct work with them
Ability to understand and correctly assess levels of need in order to formulate support plans, with clearly measurable outcomes.
Ability to understand the roles and accountabilities of other professionals in order that appropriate support can be provided to meet identified needs.
Ability to evaluate own practice limitations and identify need for support and development
Ability to contribute to the Councils aims and objectives within an overall framework that safeguards adults, promoting independence, health and wellbeing
Ability to work calmly under pressure
