We want to hear from you.
Social prescribing is a key growth area. It's giving people time, to focus on ‘what matters to me’ and taking a holistic approach
to people’s health and wellbeing. You'll connect people to community groups and statutory services for practical and emotional
support for those who have complex needs.
We are looking for a Social Prescriber to join our close-knit Community Wellbeing team in Cheltenham with a focus on surrounding
areas including Tewkesbury. This position is 22.5 hours per week on a fixed-term contract until 30th September 2024.
Are you an enthusiastic, motivated and compassionate individual who can work in partnership with Gloucestershire Health Care,
local Clinical Commissioning groups, mental health services, the Police, ambulance service, local authorities and A & E
departments?
You will be drawing on the valuable skills of the voluntary sector, social services, clinicians and community services and
agencies, ensuring the best outcome for people who use health services; where other services could assist with their ongoing
concerns.
You'll be working with people who may have long-term health conditions (including support for mental health), people who are
lonely or isolated, or who have complex social needs which affect their health and wellbeing.
There is so much variety, each day will never be the same and you will be speaking and meeting people of all ages (16+) and
backgrounds.

What experience will you need?
- We are ideally looking for someone with an understanding of social prescribing, health coaching and a person centred approach.
- You'll be able to demonstrate the ability to be a good listener, have time for people and be committed to supporting local
communities to care for each other.
- You'll have experience of working positively with people facing complex social and emotional challenges.
- You will have great interpersonal skills and a can-do approach to supporting people, community groups and local organisations.

Position: Social Prescriber - High Intensity User Lead (780)
Hours: 22.5 hrs per week
Contract: Secondment opportunity until 30/09/2024
Location: Cheltenham and Tewkesbury
Pay: £25,740 full-time equivalent. (Salary for Part-time 22.5 hours per week will be £15,422)

^Annual Leave entitlement will be pro-rata for Part-time staff. Working on public holidays due to a rota pattern will be
compensated with time off in lieu. An extra day of leave accrues for each completed financial year (capped at 5). Purchasing
additional holidays is capped at 5 per year.
^^Contracted staff only. Business mileage (45p a mile) is paid for any daily miles over and above the distance from your home to
the allocated workplace location and back. You'll need a well-maintained vehicle with valid tax, MOT and Insurance covering
business use.
