Social Prescribing Link Worker (SPLW) - Cardiovascular Recovery
The Centre is working with East London Cardiovascular Prevention Group (ELoPE) to bring Social Prescribing into secondary care for Barts Health patients. Social Prescribing is a way of linking people with non-medical support to improve their physical and mental wellbeing, connecting them with their local communities and supporting them to feel more empowered in their lives.
This project will address health inequity and support people who would benefit from:
Assistance or support for challenges in the major social determinants of health, including financial, housing and food insecurity
Being more connected to people in their local community participating in regular fun, physical activity
Emotional support
Support to engage with and access wider and multiple services
Support with wider social issues e.g. debt, housing problems, employability issues, relationship problems.
We are seeking a Social Prescribing Link Worker who will support the delivery of the Bromley by Bow Centres Social Prescribing service.
The role will provide one-to-one-person centred support to cardiovascular patients registered with Barts Health, focussing on what matters to the individual and taking a holistic approach, connecting people to local initiatives and services for practical and emotional support.
We are hoping to recruit a candidate with experience in Social Prescribing who would like to gain further knowledge about cardiovascular diseases, and the lifestyle modifications related to managing symptomatic disease.
As this is a new area of focus for us, we are ideally looking for someone who also has experience developing a project or programme (ideally within the health and wellbeing sector)
