We are seeking a dedicated and compassionate Social Worker to join a dynamic team, specialising in supporting adults aged 18-65 living in supported or residential accommodations with a diagnosis of psychosis. As part of our commitment to transformation, we are looking for an individual who can contribute to the evolution of our services by actively engaging in adult assessments and reviews of care packages under the Care Act 2014.
Responsibilities:
- Conduct comprehensive assessments and reviews of care packages for Adults with a diagnosis of psychosis, ensuring compliance with the Care Act 2014. Collaborate with service users, their families, and other professionals to gather relevant information and develop person-centred care plans.
- Actively participate in the ongoing transformation of the team. Contribute innovative ideas and best practices to enhance the quality and efficiency of service delivery.
- Work collaboratively with a multidisciplinary team, including health care professionals, support staff, and external partners.
- Engage in regular team meetings, case conferences, and collaborative decision-making processes.
- Provide emotional and practical support to individuals with Psychosis, empowering them to make informed decisions about their care. Advocate for the rights and preferences of service users within the framework of the Mental Capacity Act.
- Maintain accurate and up-to-date records of assessments, care plans, and reviews. Ensure compliance with organisational policies and legal requirements.
Requirements:
- Qualified Social Worker with a recognised social work qualification (DipSW, CQSW, CSS or equivalent).
- Registration with Social Work England.
- Experience of working with people experiencing complex mental health problems
- BIA experience ( desirable)
