Role:
- to work on Duty as part of the community Team the front door to Adult Social Care
- screening all referrals coming in to the team,
- screening safeguarding concerns and enquiries as required,
- signposting and offering advice,
- screening requests for reviews of packages of care,
- screening EMAS concerns and PPN.
The successful candidate will also be Completing duty visits and attending ARM, Safeguarding Section 42 meetings or Ward round over Teams.
Work Arrangements: Hybrid
Placement duration: 13 weeks with potential to extend
Parking: Free parking on site
Pay Rate: £32 to £34 per hour
