Job Description:

Are you an experienced Social Worker with a passion for supporting adults in need? We are seeking a motivated and skilled individual to join the Adult West Locality Team in Middlesbrough.
In this role, you will be responsible for assessing the needs of adults, developing and implementing care plans, and working collaboratively with a multidisciplinary team to provide holistic support.

Responsibilities:

• Conducting detailed assessments of adults' needs, risks, and strengths.
• Providing ongoing support and intervention to adults and their families, ensuring access to necessary services and resources.
• Collaborating with healthcare professionals, community organizations, and other relevant agencies to coordinate comprehensive care.
• Monitoring and reviewing care plans regularly, making adjustments as necessary to respond to changing needs and circumstances.
• Maintaining accurate and up-to-date records of assessments, interventions, and outcomes in line with legal and professional standards.

Qualifications:

• Qualified Social Worker registered with Social Work England.
• Proven experience working with adults in a social care setting.
• Excellent communication and interpersonal skills, with the ability to build trusting relationships with service users and their families.
• Knowledge of relevant legislation, policies, and procedures related to adult social care.
