Join the dynamic Discharge to Assess Team in Warrington Council! They are looking for an experienced Social Worker to contribute to the efficient and compassionate discharge process. This agency role through Ackerman Pierce offers a competitive hourly rate of £30 for an initial 3-month contract, with the potential for indefinite extension on a 3-month basis.
It is a very flexible working from home policy. Monday is an expected office day and the rest can be worked from home, completing visits as needed Across Warrington.
Key Responsibilities:
Facilitate timely and person-centered discharge assessments for individuals transitioning from hospital to community living.
Collaborate with healthcare professionals, families, and community services to ensure holistic and seamless discharge processes.
Conduct visits in Warrington, focusing on 3 cases per week, while utilizing a hybrid working model.
Apply your direct experience in Discharge to Assess or a minimum of 2 years' experience in a hospital setting.
Maintain accurate and up-to-date case records, adhering to data protection and confidentiality standards.
Requirements:
Qualified Social Worker with HCPC registration.
Direct experience in Discharge to Assess or a minimum of 2 years' experience in a hospital setting.
Strong understanding of relevant legislation, including the Care Act 2014.
Excellent assessment, planning, and interpersonal skills.
