Job Role: Aim to support a patient’s journey who may have complex care needs when leaving hospital or requiring local social services support.
Sirona Medical will be supplying the Social Worker on an agency contract.
This is a full time agency position.
As a Social Worker, you must have a degree in Social Work, experience working in a hospital setting, care act assessments, complext care, Social Work England Registation.
