Social Worker - Hospital Discharge

We have a fantastic opportunity for a qualified Social Worker with North Somerset Council, working within adult services on risk and care needs assessments. The post holder will be responsible for a caseload of varying complexity receiving and responding to referrals from various sources to co-ordinate Hospital Discharge, record continuing health care needs and identify and record unmet need.

This role is advertised as £33 p/h

Key roles include but are not limited to:

• Assessing and coordinating health and social care needs
• Financial assessments
• Developing, implementing and monitoring care plans
• Ensuring all service user's, especially those with disabilities, have equal access to community services

The minimum qualifications for this role are:

• Social Work qualification
• Social Work England registration
• Post-qualification work experience is desirable
