I am currently looking to recruit a band 6 Social worker to join the community access service in North Kensington. The role is Monday – Friday, 09:00am – 17:00pm, 37.5 hours per week.
Job Summary: The Community Access Service is responsible for enabling effective discharge planning, early identification of potential discharge barriers and supporting people back into their communities.
The CAS Team supports boroughs to optimize the likelihood of patients who have been admitted to their acute wards to be able to reside in the least restrictive setting. It provides a support and challenge function to the current over reliance on high support settings including rehabilitation units, supported living placements and residential / nursing placements, unless clinically indicated.
All core CAS frontline workers will work alongside the discharge teams / bed managers in each borough,, and also closely with the rest of the acute inpatient teams, Home Treatment Teams, Community Mental Health Teams / Hubs and the local Occupational Therapy lead to deliver the service.
