A fantastic opportunity has arisen for a Social Worker to join the Mental Health Team.
 
As a Social Worker you will receive and manage incoming referrals, undertake a range of assessments and reviews, manage Safeguarding concerns and enquiries whilst ensuring a needs led approach, ensuring a focus on community assets in partnership with service users, carers and other professionals and where applicable arrange appropriate support packages that address the eligible social care needs identified.
 
Responsibilities will routinely include Social Care assessments for adults and carers, reviews, care and support planning and continuing health care decision support tools along with adult safeguarding enquiries.
 
This will require accurately maintaining client records and ensuring that these records are logged correctly on to the departments IT system along with strong report writing and communication skills.
 
You will also be supported with regular supervision and performance management sessions.
 
Along with a relevant qualification you should have relevant knowledge and experience in Adult Social Care either in a formal or informal capacity and be able to fully meet the requirements of the post and cover the whole geographic area of the City either by possession of a current and valid dri
ving licence or by access to public or other transport.
