The Role:
The Community Hospitals Team have an exciting new opportunity for a Social Worker. We are a large supportive team who are based primarily on the wards in the Community Hospitals and work alongside our health colleagues as well as our colleagues in the Acute Hospitals. The Team covers all the Community Hospitals across the county and is part of the wider Urgent Care Service, so we provide lots of opportunity to develop and gain experience as you progress in your career.
The vacancy is based within the Community Hospitals in the South of the County primarily at Pershore Community Hospital. While we will always limit the amount of travel as a Countywide Service there will be occasions where a flexible approach is required as you may also be needed to support the Countywide Wrap Around Service. The Wrap Around Service is a is short term 24hr Care provided within the person's own home. The scheme is well used so the flow through it is key.
As a Social Worker you will be expected to have a working knowledge of the Mental Capacity Act and Care Act ensuring that services are the least restrictive and provided in line with Choice of Accommodation Policy. You will need to be able to identify Adults who may be at Risk or Abuse and be prepared to work with the Safeguarding Team in cases where you are identified as an Enquiry Officer.
The post is predominantly aimed at Social Workers who have some experience post completion of their ASYE, however we will accept and consider applications from Social Workers who are earlier in their career, but have completed their ASYE.
The Team meets regularly in Team Meetings where we have guest speakers and in smaller cluster meetings with ASWP’s where Social Workers can reflect and share aspects of good practice and learning. Social Workers receive regular Supervision and support to ensure you can attend training and participate other aspects of Social Work Development.
As a Qualified Social Worker, you will need to be registered with Social Work England.
