Role: Qualified Social Worker

• Manage duty desk within the older adult CMHT
• Complete assessments in the community following new referrals 
• Hold a small case load for care coordination 
• Role is office based  but includes home / care home visits to complete assessments.
• Flexi working can be negotiated.
• Worker must have access to a car.

Contract - Working hours - 09.00 - 17.00
