REACH Team, Lewis Place, Perth

REACH is an exciting initiative created to provide services for young people who are Looked After or at risk of becoming Looked After and therefore “on the edge of care”. REACH promotes a multi-disciplinary approach to providing community-based support to young people and to their families and will reduce reliance on traditional residential care options for vulnerable young people.

Young people who enter care during their teenage years traditionally spend considerable periods of time in residential care, separated from their family and local community. We know that this is detrimental to achieving their potential and in meeting key outcomes. We have recognised the need to transform our approach to supporting vulnerable young people and their families and this service has been specifically designed to do so in a more integrated, co-ordinated way.

Do you want to make a difference? If so, within this role you will deliver a frontline service to some of the most vulnerable young people across Perth and Kinross. These will be young people, aged 12- 18, who are participating in harmful behaviours; at risk of child sexual exploitation and/or are identified as being " on the edge of care".

Working within a multi-disciplinary team including Senior Social Care Officers, Speech and Language Therapists, Clinical Psychologist, Outreach Teacher and Youth Worker; you will be responsible for managing a small caseload and will design and deliver 24/7 packages of support to improve outcomes for young people.

In this role, your working pattern will be Monday-Friday 8.45am – 5pm.

You will undertake statutory responsibilities related to Looked After young people. This will include assessments of need/ risk.

You will a lead role in the development, co-ordination and review of care plans and will undertake direct work with young people and their families.

You will provide written reports and represent departmental recommendations in a range of forums such as Children’s Hearings, Child Protection Case Conferences and Looked After Review Meetings.

Ideally, you will have post qualifying experience within a statutory childcare or youth justice setting.

Experience in the use of START:AV and AIM3 assessment frameworks is desirable but not essential as training will be provided.

You will have a degree in Social Work or equivalent as recognised by the Scottish Social Service Council.

You must have a valid UK driving licence as this post requires you to travel to a variety of locations across Perth and Kinross.

This post may be suitable for candidates who have recently completed their Social Work Degree but have not yet graduated. In this circumstance, a Social Work Assistant contract will be offered until you receive your qualifying certificate and are eligible to register with the SSSC as a Social Worker.
