Job description
Focus on refining and reproducing the delivery of specialist pharmaceutical care to a case load of older patients within the Care Home setting.Work closely with the Consultant Pharmacist Older People, Consultant Geriatricians, secondary care clinical pharmacy teams, secondary and primary care multi-professional teams GPs, specialist nurses, AHPs and community pharmacists to deliver high quality specialist pharmaceutical care to older people within the Trust area.Work across a range of areas including Care Homes and the acute hospital setting.

5 years post registration experience including 3 years in hospital of which 2 years must be at band 7 or aboveMPharm or equivalent and registered with or eligible to register with the Pharmaceutical Society of Northern Ireland (PSNI)
