Job description

Planning and Organisational Duties The named School Nurse will be responsible for co-ordination of the School Health Plan with their named schools on an annual basis. Contribute to an integrated Public Health Nursing Service by linking to education settings and other partner agencies Lead on the recommended health assessment and reviews including School Health profiles as required. Lead and coordinate a core programme of interventions according to agreed procedures, ensuring that Trust targets are met. To lead, organise, coordinate and participate in the delivery of the national immunisation programme for school aged children. Plan, implement and evaluate interventions according to changing health needs to improve health outcomes. Consult, advise and educate children, young people and families to enable them to effectively manage their health needs. Where necessary carry out home visits to children and families advising on all aspects of health as appropriate. Working in partnership with other key stakeholders to safeguard and protect children and young people. To contribute and take part in regular team meetings and contribute to multi-agency groups as appropriate. Working collaboratively to support children and young people where there are identified health needs, or where they are in the child protection system, providing therapeutic Public Health interventions for the child and family Provide training where necessary to young people, parents, teaching personnel and other professionals. Provide signposting to other services and agencies, referring and liaising appropriately. Take and act upon referrals from other agencies and services, as appropriate. Provide a confidential support and advice service; that is easily accessible, for young people within a high school or other venue on a regular basis. Organise, facilitate and participate in health promotional and health educational activities in schools and community settings, working in partnership with school staff and with other statutory and voluntary agencies. Utilise opportunities to promote healthy lifestyles, positive mental health and the Public Health agenda. Ensuring children not in employment, education or training, or children educated at home receive the universal offer. Undertake nurse led immunisation programmes in schools and other venues following patient group directions, actively promoting the National and Local Immunisation Programmes. Complete PDRs as required to designated named staff within the team. Ensure performance data is collated in a timely manner. Clinical Maintain active status on NMC register acting in accordance with the professional code and guiding documents In adherence with Revalidation Framework. Develop a working knowledge and experience of IT systems Maintain contemporaneous, factual records in accordance with Trust guidelines and policies. Be aware of current working practices in accordance with Trust policies and procedures. Manage their time effectively to ensure prioritisation of workload in order to achieve agreed deadlines. Be responsible for own professional updating and keep abreast of current research based practice. Participate in study days as appropriate and be aware of current developments and research within the area of practice. Observing the complaints and disciplinary procedures in accordance with the Trust policy. Report sickness absence and return relevant forms to line manager. Maintain portfolio of professional development. Prescribe safe, effective and appropriate medication as defined by current legislative framework and in line with organisational policies and only prescribe within their scope of practice, area of expertise and level of competence. Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national, local and organisational guidance and formularies. Leadership Provide supervision, where appropriate to support the skill mix team Undertake appropriate delegation within team. Ensure service information is disseminated to all team members in a timely fashion. Contribute and lead in service improvement initiatives. Support other School Nurses and staff within the team, including the induction of new staff members. Ensure effective learning experience and opportunities to achieve learning outcomes for skill mix team. Develop specialist knowledge and expertise in an area of practice appropriate to service needs and, act as a resource to colleagues and other professionals in the chosen area e.g. Asthma, Behaviour Management. Allergy etc. Develop and implement policies, procedures and guidelines relevant to own area of work. Provide advice where services are developing. Taking the lead in developing effective partnerships and acting as advocate delivering change to support improvements in health and wellbeing of school aged children. Ensure and be able to evidence that the experience and involvement of families, carers and children will be taken into account to inform service delivery and improvement. Collate local information utilising a school health profile to help inform the Joint Strategic Needs Assessment. Research and Audit Identify and undertake clinical audit, evaluation and dissemination of results through presentation to professional groups and publications, in order to improve effectiveness of care and ensure effective and efficient use of resources. Participate in benchmarking, developing and implementing action plans resulting from these activities. Disseminate audit findings through presentation to professional and other groups. Lead on clinical audit within the team. Supervision and Development Identify and make known any training needs through the Personal Development Review (PDR) process. Provide mentorship and preceptor ship to students and newly qualified staff as required. Support the development of the team by teaching and assessing junior members of the team. Undertake mandatory and statutory training and updating as required by the Trust. Actively participate in the Transformational Supervision sessions using reflective practice. Communication Communicate effectively with individuals, families and professionals in planning and delivery of care; demonstrating empathy and reassurance in difficult and highly sensitive situations. Utilise a range of communication methods with colleagues and managers to include electronic communication. Work in partnership with school staff, health colleagues and other agencies e.g. Education and Social Services. Communicate complex information and provide specialist advice to clients/carers/professional agencies. Maintain contemporaneous and accurate WWL records observing the Data Protection Act 1994, maintaining confidentiality, in accordance with NMC and Trust guidelines Responsibility for Health & Safety Compliance with the Health & Safety at Work Act 1974 the post holder is required to fulfil a proactive role towards the management of risk in all of their actions. This entails the risk assessment of all situations, the taking of appropriate actions and reporting of all incidents, near misses and hazards, and a statutory duty of care for their own personal safety and that of others who may be affected by their acts or omissions. Work Circumstances Monday to Friday between 0800-1700.
