KEY RESULT AREAS Develop partnership working relationships with staff which enables quality issues to be discussed. Ensure delivery of quality standards in respect of waiting times.. Develop effective networking relationships with other departments. Work with others to aid the implementation of nurse-led clinic sessions and colonoscopy sessions in the department (GGH sites, other UHL sites,Alliance), ensuring facilities are available for data collection and that the process is understood by all concerned. Be responsible for care delivery to meet the needs of all patients identified by the screening programme at any stage of the screening process. Perform health assessments, develop and record care plans and monitor their implementation, making adjustments as necessary. Ensure Clear communication to enable continuity of care and seamlesstransition from screening into primary, secondary and outpatients caresettings and to multidisciplinary cancer team meetings when necessary. Responsible for dissemination of high quality and understandable information, ensuring equity of care by overcoming barriers such as language and disability, to assist patient choice.Use appropriate communication skills when supporting patients who receive bad news. To increase patient safety and reduce high levels of anxiety that are known to occur in screening patients by providing timely information, appointments within national guidelines, support at colonoscopy, and telephone support to patients and relatives at all stages of the screening process and across all participating hospitals. Populate the data collection for the bowel cancer screening programme. Undertake research, clinical audit and benchmarking to improve clinicaleffectiveness and help reduce bowel cancer mortality or morbidity in the future Service Delivery & Development Provide advice, information and support for those making direct contact with the screening centre. Responsible for delivery of specialised care to patients identified by thescreening programme and meet the needs and wishes of patients arising as a result of:Having been called to participate in screening.Having been identified as positive on screening.Needing to be assessed as fit to undertake screening colonoscopy oralternative diagnostic investigations.Needing to be able to make informed decisions prior to undergoingscreening colonoscopy or alternative diagnostic interventionsHaving been identified as requiring further intervention, treatment,surveillance follow up or discharge. Ensure timely access to screening colonoscopy or alternative diagnosticintervention and seamless transition to agreed pathways in primary,secondary, tertiary or outpatient care settings, during all stages of thescreening pathway. Working with the Programme Manager and Lead Specialist Screening Nurse to ensure that there is appropriate planning and tracking to ensure that patients progress along agreed pathways and are seen within agreedtimescales. Establish effective links and attendance at MDT with local colorectal cancer multidisciplinary teams in order to ensure that where cancer is detected thereis a timely presentation and transfer of patients to the MDT at location agreed with patient. Provide highly specialised advice which contributes to the diagnosis, care or education of patients in the BCSP. Promote good working relationships and collaborative working with the Interprofessional team, within the unit, Trust and other BCSP Treatment Centres. Analyse complex patient conditions, determining whether condition is suitable for screening or alternative diagnostic interventions and when to refer to other professionals if they fall outside the screening pathway. Provide health promotion for the screening programme in liaison with the CCGs to ensure equal access to health promotion and information relating to the BCSP for all sections of society. Perform comprehensive assessment of patient needs, plan, implement and evaluate care appropriate to the needs and wishes of the patient and in accordance with the BCSP and local policies, procedures and protocols. Assist in the setting of clinical standards within the trust to improve the quality of patient care. Evaluate service delivery, identify areas for improvement and initiatechange. Contribute to team meetings and other appropriate meetings within andexternal to the Trust to influence and improve service delivery. Contribute to the business and strategic planning of the BCSP. Participate in the management and development of the BCSP sharinginformation and ideas that promote quality and value. Provide access to counselling and psychological support for team members, patients and their families to create a caring environment. Governance Actively promote awareness of infection prevention and control using best practice to reduce the potential for cross infection. Promote culture to ensure patient care is always of the very highest standard Ensure the principles of clinical governance are fully incorporated into the operation of the unit. Ensure health and safety is maintained in all clinical areas, using riskmanagement initiatives, dealing with critical incidents and promoting andproviding a safe environment for staff and users. Work in accordance with the BCSP, hospital and department policies,procedures and guidelines. Have an awareness of potential differences topolicies and guidelines. Contribute to policy making and clinical guidelines internally and externally, necessary to support the specialist nursing service. Participate in ongoing data collection and quality assurance for the BCSP,service feedback and audit. Populate and maintain the BCSP database, including all interim systems. Ensure patient records for the screening services are completed to include data on treatment, staging and outcomes provided by the MDT. Support the BCSP Matron/Programme Manager in the development ofperformance reports to the National Screening Office, strategic groups and the wider Health Community. Undertake specialist education training as required that will allow health care to be delivered to the user through new ways of working. Patient/Customer Service Forge successful relationships with other health care professionals and nonclinical support staff to improve overall patient experience. Be pro-active in seeking out the views of users in order to appreciate andacknowledge satisfaction or areas for improvement. Deal with concerns or complaints promptly with tact, diplomacy and attention to detail, both verbally and in writing Work with colleagues to review patient information literatureContinuing Education, Professional and Personal Development Undertake the Trust Corporate and Directorate specific Induction andcompetency Programmes appropriate to role. Take responsibility for ensuring self and all staff comply with mandatorytraining and NMC professional re-registration requirements. Actively participate in life long learning and personal development through appraisal and Knowledge and Skills Framework (KSF) development plans. Demonstrate continuous evaluation of practice and make changes whereappropriate, within the defined speciality. Use Education forums and Networks proactively sharing good practice and innovation with others. Liaise with other groups and assist with the delivery of multi-professional and inter-professional learning. Provide specialist education teaching, learning and mentoring for pre and post registration students when required and actively contribute to the delivery of the Universities curriculum. Act as a clinical expert and use own knowledge to develop and enhance the development, skills and competence of others. Participate in presenting at local and national conferences jointly with the rest of the team both locally and nationally to promote the implementation of the bowel cancer screening programme. Research Demonstrate a high level of commitment to and enthusiasm for research,audit and education in the clinical area. Actively participate in audit and research and benchmarking activities toensure effective safe and evidence based practice. Health and Safety Monitor health, safety and security of self and others and promote bestpractice in the ward area. Ensure own actions assist with maintaining best practice and Standards for Better Health. Carry out duties in accordance with the Health and Safety at work Act 1971.Move and handle patients and goods in ways that promote the health of the patient and care team and are consistent with legislation. Be actively responsible for the maintenance of a clean and safe environment adhering to infection control policies, procedures and best practice.
