Job summary
We have a new and exciting opportunity for a Band 5 Speech and Language therapist with an interest in working with people with neurological conditions to join the well-established Community Neuro Rehab Team (North) in Horsham.

We are looking for:
  • A Speech and Language Therapist who has an enthusiasm and interest in neuro-rehabilitation
  • A Speech and Language Therapist who is competent to work autonomously as well as part of a team.

The Community Neuro Rehab Team (North) is an established multi-disciplinary team consisting of OT's, physios, SLT's, dietitian, social worker, specialist nurses and rehab support workers and technicians. The successful candidate will be working alongside and supervised by a specialist band 7 SLT. There are regular opportunities for peer supervision and professional development within the wider adult SLT service.
The team is welcoming and friendly with staff who are passionate about neuro rehab and offer empathy and high standards of care to patients.
Main duties of the job
1. To independently organise and manage a complex caseload.
2. To make differential diagnoses of communication and swallowing disorders and specialist clinical decision independently, following specialist assessment.
3. To provide specialist evidence-based intervention and evaluate clinical outcomes to ensure clinical effectiveness using agreed evaluation procedures.
4. To be responsible for clinical administration and record keeping related to the specialist caseload according to Departmental and Trust policies relating to the format and security of patient records.
5. To liaise with referring agents, medical and nursing staff, multi-disciplinary team members, relatives/carers, and relevant others to ensure effective continuity of care, establishing a high-level professional relationship engendering confidence in the SLT service.
6. To identify swallowing and/or communication risks as they arise within clinical settings and within reflective practice and in supervision, following relevant Trust, departmental and professional risk management, and incident reporting guidelines.
7. To work with the MDT advising on management of specific SLT related clinical issues, to collaborate in the development and co-ordination of team objectives.
8. To share information with others, observing information governance principles and guidelines.

Job description
Job responsibilities
The team serve the population of Horsham, Crawley and Mid Sussex and therefore access to a car with business insurance is an essential requirement of this post.
  • To identify individual training and development needs through supervision and appraisal, developing specialist clinical skills through attendance at appropriate continuing education events and independent study as identified in the PDP; and to attend mandatory training as required.
  • To participate in providing training and support to assistants, generic workers, and volunteers and to other SLTs, other professionals, relatives/carers, and relevant others and to participate in evaluating the effectiveness of the training given.
  • To provide appropriate and timely client related correspondence according to Trust, professional and service requirements including the requirements of other relevant and involved agencies
  • To provide accurate and timely information and statistics pertaining to caseload pressures to the SLT clinical lead to facilitate service management and planning.
  • To comply with the standards and priorities laid down by the relevant managers and in line with RCSLT Communicating Quality 3, the policies of SCFT and clinical guidelines relevant to the specialist area.
  • To reflect on clinical practice within supervision and with peers, identifying strengths and needs; seeking advice and support as required and providing evidence of sound clinical judgement, implementation of evidence-based practice and use of outcome measures.
  • To be responsible for the care of equipment and resources; to monitor equipment and resources needs and to request new items when necessary, through line managers.

Qualifications
Essential
  • Degree (or recognised equivalent) in Speech and Language Therapy
  • Registered member RCSLT and HCPC

Experience
Essential
  • Relevant experience of working with adults with acquired communication and/or swallowing difficulties at undergraduate/ post graduate level
  • Experience of multidisciplinary working within acute and/or community settings, including understanding of the roles of other professionals relevant to caseload

Skills and knowledge
Essential
  • Knowledge of a range of assessment tools relevant to the patient group (acquired communication and swallowing disorders)
  • Evidenced based knowledge of a wide variety of treatment techniques and strategies relevant to the caseload (adult acquired communication and swallowing disorders)
  • Willingness and ability to be a good team member
