This is a community-based post and a car driver is essential for carrying out home visits to patients’ homes and care homes.
We are looking for a Support Worker with previous experience of working with frail patients in a health or care setting. The ideal candidate will be able to work independently, as part of a large multi-disciplinary team and have excellent communication skills. The post holder will have an interest in, and the skills to develop the role as it is a new post.
The post holder will be required to assess and provide first line advice for patients with nutritional, swallowing and communication difficulties. This will involve following assessment and treatment protocols, referring on to trained clinicians when appropriate. Full training and support will be provided. You will have a varied day-to-day role, including independently visiting patients in their own homes, and supporting the clinicians as needed.

To provide Therapy assessment and treatment of patients referred to the service designated by a qualified Speech and Language Therapist or Dietician .
Assesses and treats patients without direct supervision according to team/department standards and protocols
A flexible post working across our integrated frailty services including Urgent community response service and same day emergency care (SDEC) The main role will sit in hospital at home team based at Milford hospital .
you will be required to work at main Royal Surrey hospital site on occasions, for initial training and for on going support and clinical supervision from the Dieticians .
