Job description

Eleanor Frailty unit is a 12 bedded ward with three bays and mainly deals with frail patients who need less than 72 hours period to be discharged .We aim to get frail older people seen by multidisciplinary team and help them get better sooner and healthier.
We are seeking enthusiastic, highly motivated nurses to join the team at Northampton General. You must have excellent communication skills and demonstrate a keen interest in Frailty , as well as the desire to develop your knowledge and skills within this area.
The department offers a wide range of education and development opportunities for the right person and excellent support from a mentor and senior nursing team. We offer a comprehensive supernumerary period with support from a preceptorship team.

To deliver exemplary nursing care to all patients presenting to the Frailty unit at Northampton General Hospital whilst working within a team of dynamic and motivated healthcare professionals.

Role and responsibilities:
To act at all times, with regard to the Nursing and Midwifery Council Code of Professional Conduct.
To be aware of and adhere to the Trust’s policies and guidelines.
To be responsible for assessing, planning, implementing and evaluating patient care maintaining the highest standards possible at all times.
To liaise with members of the health care team and patients’ families to discuss care appropriate to individual patients.
To have a patient focussed approach to care, ensuring that each individual patient has the best care possible.
To be motivated to acquire new skills and knowledge.
To be responsible for the accurate documentation of nursing information and be aware of the necessity, at all times, for effective written and verbal communication.
To utilise the best practice available, utilising research and/or evidence based practice at all times.
To support both senior and junior members of staff and students, demonstrating commitment and loyalty to the profession and the Directorate.
To help create a positive working environment for all members of the health care team, promoting positive working practices at all times.
To attend and participate in department meetings, helping to implement change as required.
To attend a regular performance review with the Ward Sister, Charge Nurse or Senior Nurse.
To assist in the ordering and storage of supplies ensuring that the most cost effective use is made of resources.
To accept personal responsibility for identifying own educational needs and professional development.
To assist with teaching, mentoring and the supervision of staff, thus actively participating in maintaining a positive clinical learning environment.
To advise patients, where appropriate on the promotion of health and the prevention of illness and accidents.

The post holder may be required to carry out other relevant duties as required.
The post holder will adhere to the duties specified under the Staff Responsibilities of the NHS Constitution in their day to day work and behaviours.
The post holder will be expected to aspire to the Values of the Trust in their day to day work and behaviours in order to support the Trust in achieving its Vision.
The post holder will adhere to, at all times, any Professional, NHS Code of Conducts and legislation relevant for their area of work.
The post holder will make themselves familiar with, and adhere to, at all times, the policies and procedures of the Trust, and their area of work.
The post holder will be expected to work to any Corporate/Division/Directorate/Department objectives and standards in order to provide an acceptable level of service.
The post holder will be expected to undertake training, including mandatory and role specific training, relevant to their role and ensure it is renewed as required. This job description reflects the present requirements of the post and it does not form part of the contract of employment. As the duties of the post change and develop the job description will be reviewed and will be subject to amendment, in consultation with the post holder. It is the Trust’s aim to reach agreement on reasonable changes, but if agreement is not possible the Trust reserves the right to effect changes to the post holders job description after consultation with them. Appropriate notice of such changes will be given.
