Responsibility for planning, co-coordinating and reviewing own caseload in conjunction with the team. Ensuring that workload is appropriately prioritised and managed. This includes children who have moved into the area, have medical needs, faltering growth, recall due to developmental needs and children on plans. Carry out allocated mandated checks (part of HCP 0-5) as delegated by a Health Visitor e.g., 9-12 months developmental check, 2-2.5 year review. Undertake holistic health assessments and plan, implement and evaluate evidence based nursing care for individual children and their families. Support the development, planning, implementation and evaluation of health education and promotion packages using evidence based materials and methods. Where appropriate, contribute to multi agency processes to assess and develop care plans to support children health and wellbeing e.g. children who are Looked After, in need of safeguarding, in receipt of Early Help. Have a good knowledge of local services in health and partner agencies and where appropriate, liaise and refer a child or their parent/carer to multi-disciplinary teams and other specialist services. Where appropriate, take on the role of lead professional to coordinate delivery of care plans for children e.g. Children on Early Help plans Working in partnership with the multi-agency team provide clinical advice, information and support regarding children with long term conditions, disability or specific health problems to minimise negative impacts on their lives and promote health gain and social inclusion to the children, family and carers. Support the development, planning, implementation and evaluation of health promoting activities with individuals and groups, utilising evidence-based materials and methods.
