Job description

We are looking for a new Associate/trainee Community Clinical Nurse Specialist - Palliative Care to join our Hospice Community Specialist Palliative Care Team. The Hospice consists of an 18 bedded in patient unit, a Day Centre and a community team maintaining highest standards of palliative care with in multidisciplinary environment. Services are provided for people with cancer, HIV and other life threatening illnesses and for their significant others.
The Team of Community Clinical Nurse Specialists in Palliative Care, working from St John's Hospice, offer specialist palliative care to patients diagnosed with cancer, or any life limiting illness, at home throughout Westminster . The team work closely with local primary healthcare teams including regular attendance at General practitioner (GP) palliative care meetings and joint visits with GP's and District nurse teams. The Community Clinical Nurse specialists also take part in training and education programs for nursing home staff and community health and social care professionals in the locality.

What are the main responsibilities of the Associate/trainee Community Clinical Nurse Specialist - Palliative Care Role?
  • Work within the NMC Scope of Professional Practice and Code of Conduct
  • Identify professional objectives and developmental needs
  • Work under the direct supervision of one of the Community Clinical Nurse Specialists in Palliative Care in maintaining a caseload of patients
  • Attend regular clinical team meetings, locality meetings and Multidisciplinary team meeting and be able to discuss the management of patients with the clinical team
  • Assist the Community Clinical Nurse Specialist team in the continuous assessment and evaluation of the needs of patients and families, recognising physical, psychological and social needs, and agreeing ways in which these might be addressed.
  • Work with the wider clinical multidisciplinary team to provide ongoing assessment of current patient needs, effective symptom management and monitoring and evaluation of effects of prescribed treatment.
  • Demonstrate effective communication with colleagues at St John's Hospice , acute and community healthcare professionals liaising with other palliative care/oncological teams/units and other specialist professional teams in order to facilitate continuity of care for patients with life limiting illnesses.
  • Ensure accurate written and computerised patient records are maintained including inputting of patients onto the electronic end of life care register Universal Care Plan
  • Demonstrate skills in working with families facing loss and identify any particular risk factors that are of concern and communicate these to the Hospice Social Work and Bereavement Service.
  • Participate in clinical audit as part of the hospice audit programme

  • NMC Level 1 Registration
  • Working towards a Graduate level or equivalent qualification in relevant discipline
  • Advanced communication skills training is desirable
  • Advanced physical assessment skills training is desirable
  • Good communication and interpersonal skills
  • Demonstrate experience in complex communication in a variety of settings
  • Teaching skills
  • Report writing and presentation skills
  • Motivated and enthusiastic approach to Specialist Palliative Care
