Salary An exciting opportunity has arisen within Forth Valley Royal Hospital, based at Larbert.
Ward A11 has an exciting opportunity to develop and improve the journey of patients who attend the acute receiving areas of the hospital who have complex discharging needs. This post would be ideally suited to a highly motivated nurse, who has interest in multidisciplinary working that can manage the complexities attached to patients’ care including effective discharge planning skills.
As part of a multidisciplinary team, including nursing, AHP staff, psychiatric liaison, consultants, discharge coordinators, community teams and integrated social services the post holder will have responsibility for assessment, care planning and coordination of the patients to expedite early intervention and planning of discharge.
The successful applicant will have excellent communication skills and have an extensive networking knowledge of staff and services across acute care and health and social care partnerships.
A genuine interest and clinical knowledge of complex discharge planning is essential. You will be expected to work a flexible work pattern.

We have various positions available, up to 37.5 hours per week available. Consideration will be given for part time hours.
