We are currently looking for a Supported Pathway Officer to work in Southwark. The role is Monday to Friday. Full time, shifts are 8-4 and 12-8. The postholder will carry out crisis intervention assessment of a Rough Sleepers needs and then work with partners to develop a plan to meet customers' needs, agree their pathway route and move them on to suitable accommodation within the timeframe agreed.
Accountabilities:
Arrange bookings for new residents, coordinating, interviewing and assessing risk and carry out Risk Management and SOVA assessments for all new residents and inducting them to the Health and Safety requirements of the accommodation
Carry out a strengths-based assessment with each client. Work to understand and advocate for the individual's personal preferences and goals, whilst managing client expectations.
To deliver assertive, proactive, immediate, responsive, flexible and creative support solutions to help clients to maintain a tenancy.
Contribute to the day to day delivery of the hostel service, in accordance with all agreed objectives, procedures and policies, including required health and safety inspections, drills and records. Carry out room checks to monitor the health, safety and well-being of the rooms and clients and where necessary, provide hands on support to encourage and help residents to keep good care of their room and wellbeing.
