Part Time Referral Coordinator

 

Swan Care North – Colchester

 

£17,740

 

30 hours per week – Minimum 4 days a week

 

 

We are looking for a Referral Coordinator to join our team of 6 at Swan Care North, Wellington House in Colchester. You will be responsible for the Referrals process and ensuring it runs seamlessly and meets the SSL standards in all customer care areas.

 

We are looking for candidates with a willingness to learn, who can be resilient and have a calming nature, as the service can get very hectic.

 

We provide a comprehensive induction programme as well as on-going training to develop new and existing skills and encourage progression as well as ongoing support from your line manager. Sanctuary has their own Learning and Development Academy, who provide a wide range of training courses, open to all.

The role of Referral Coordinator will include:

  • Working in partnership with management to ensure data is correct and KPI’s are met
  • Providing telephone support, information and guidance to staff and external stake holders
  • Allocating care staff using the care planning system and ensuring a thorough handover of any actions to carry out
  • Working with Health and Social Care Partners to assist with the transition for longer term care or reablement
  • Working with stake holders and partners to ensure safe transfer of care and participate in team meeting
  • Attending training as required to enhance your skills

 

Skills and experiences:

  • Similar experience in a similar role
  • Good communication skills both written and verbally with the ability to relate to customers
  • Knowledge of CQC standards  
  • Ability to work individually and as a team whilst liaising with other professionals
  • Understanding Equal Opportunities and Anti-Discriminatory Practice issues
  • Knowledge and skills of IT systems, word, excel etc.
