Job description


Walsall Healthcare NHS Trust provides both acute and community services to the Walsall and surrounding areas population. We have a high commitment to providing high quality care that is patient centred.
We are looking for a highly motivated individual to support and be responsible for the Urgent Community Response (UCR) development across community, multi-disciplinary teams within the Walsall Borough.
You will be working across a number of front-line services within the community pathways, predominantly working within the newly resigned UCR team but having strong links of supportive collaboration with the Integrated Front Door Service, the enhanced case managers for care homes, Care Navigation Centre, Virtual Wards and locality-based teams.
As part of your on-going continual professional development, you will be required to build and maintain a portfolio of evidence of your practice supporting as professional developing into an Advanced Clinical Practitioner and the service which they are part of. This will be reviewed regularly by the Community Lead’s as the community services evolve into a fully integrated MDT across the borough.
Applicants should be a Band 6 or 7 with significant experience in Acute or Community settings.
Successful applicants will be part of a 24/7 pro- rota and will be expected to contribute to all aspects of the working model across 7 days a week.
The post holder will be responsible for the leadership and establishing a nurse led equitable service for patients throughout the Walsall Healthcare NHS Trust whilst also working closely with other outside agencies - including social services, acute hospital and voluntary organisations. The post holder will work to strengthen clinical and professional leadership and inspire colleagues within the multi-disciplinary team to improve and maintain evidence based best practice whilst developing their own professional practice.
The post holder will be responsible for delivery of advanced level of care to adult patients who have long term conditions and at risk of hospital admission through an urgent community response. Within the role there will be a significant focus on patients within their own home to avoid hospital admission.

Main Responsibilities
• Use advanced skills and expert knowledge to assess the physical and psycho – social needs of a client group with complex multiple long term conditions, instigating therapeutic treatments, including prescribing, based on best available evidence in order to improve health outcomes.
• To provide advanced knowledge of patients with long term conditions and sub acute clinical conditions to ensure best practice across WHT exploring and implementing ways of preventing unscheduled admission for patients into secondary care that meet the 9 UCR response guidance.
• To make and receive direct referrals to and from nurses, the ambulance service, consultants, general practitioners and other members of the multi-disciplinary team e.g. podiatrists, physiotherapists and occupational therapists.
• To facilitate advanced care to patients with managed care needs, acting as an accountable role model through the demonstration of evidence based practice.
• To work with and provide clinical support and highly specialist advice to associated nursing staff across the integrated health and social care teams.
• To set up and participate in the community virtual ward models as established and its associated systems which include working in partnership with medics, nursing and allied health care professionals, information technology providers and performance teams.
• To establish and maintain professional relationships with all relevant professionals and support them in establishing appropriate services and interventions that will meet the needs of patients, families and carers.
• To develop professional expertise through practice and continuing professional development at a local, national and international level.
• Contributing to maintenance of the Community services NMP database as part of the overall Trust NMP database, provision of information and communication between the Trust and community prescribers
