Job description

Clinical Quality and Safety Carries and continuous responsibility for a defined patient caseload of patients, acting as the patient advocate and supporting their environment in decision making about their care. Develop and provide Nurse led Endoscopy Sessions in collaboration with the Clinical and Nursing staff. Perform diagnostic and therapeutic Endoscopy procedures, identifying clinical abnormalities and acting on that information within agreed protocols. Clinically assess patients and undertake planned investigation within agreed protocols. To undertake all appropriate correspondence for patients in their care with primary and secondary care colleagues. Refer patients, whose fitness for a procedure may be in doubt to the referring consultant or medical practitioner for further advice and management. Reviews, interprets and acts upon both laboratory and near patient test results which may effect change to individual care plans, liaising with appropriate Medical staff when necessary. To arrange follow up appointments when appropriate to monitor and evaluate client health/illness. Obtain consent in accordance with Trust guidelines from patients undergoing endoscopy. Maintain accurate records on the electronic recording system to ensure continuous audit is captured. Provide support to patients/relatives regarding procedure and the outcomes and any further treatment that maybe required. Assist in the development of local clinical guidelines, protocols and patient pathways. Able to prescribe sedation and other medications necessary to undertake endoscopic procedures. Promotes programmes of health education and health promotion and involves patients and carers appropriately in decision-making. Communicates highly complex, sensitive, distressing or unwelcome and potentially contentious condition related information to patients with empathy and understanding. Communicates effectively with hospital medical staff, general practitioners and nursing staff giving highly specialist support and advice in the care and treatment of patients within their speciality and provide expert professional advice for other members of the healthcare team and patients as required. Clinically and professionally expected to autonomous decisions on a daily basis including provision of advice to junior staff in clinical decision making. Is able to effectively manage situations where conflicting views or difference of opinions arise in a calm and professional manner. Works within national policy frameworks relevant to the patient group and speciality area i.e. Child Protection, Vulnerable Adults, and Mental health Act. Actively participates in all aspects of the clinical governance process. Takes a lead role in promoting infection control, environmental safety and risk management in order to and minimise risk to patients, self and other staff at all times. Supports the implementation of local/national targets in conjunction with relevant members of the multi-disciplinary team. Participates and supports implementation of national guidance relevant to service i.e. National Service Framework, NICE guidance, JAG, etc Actively promotes the effective and economic management of resources available within the area of responsibility. Takes the lead in the development and evaluation of standard operating procedures in relation to specific care systems. Supports the Lead Nurse and Directorate Manager in the assessment, procurement, implementation and management of equipment with the Endoscopy unit. Undertake and carries out clinical triaging of patients referred for both upper and lower endoscopy, and undertakes other clinical roles as required by the Trust to develop services for endoscopy patients. Service Development / Improvement Undertake regular audits in specialist area using research methodology in order to implement changes in practice and thereby improve patient care. Takes an active role, working in conjunction with the Lead Nurse, Clinical Lead, managers and specialist colleagues including Medical Consultants in the speciality, to propose, develop and implement service change and improvements to enhance the clinical service. Participates in the writing and reviewing and implementation of policies and protocols in relation to specialty area of care, including where appropriate, those which may impact on other professional groups. Actively contributes to the strategic management of the specialty supporting the development of the service, patient information, care pathways and user involvement. Actively identifies areas for service improvement as part of their directorate clinical governance agenda. Develops, implements and evaluates new initiatives. Management Maintain membership on working patients at local and national level. Takes individual responsibility and recognises corporate responsibility for leading and supporting the clinical team in NHS Agenda i.e. Investors in People, Learning together Working Together, Patient & Public Involvement, and relevant White Papers. Providing leadership by identifying and promoting the need for change, motivating the clinical team to embrace change and manage the change process. Assists in the development, interpretation and evaluation of clinical policies, protocols and clinical pathways to meet the needs of the specific patient group. Maintains update, accurate and comprehensive patient health care records. Participates in the investigation and response to formal and informal complaints and incidents in line with relevant Trust policy. Provides positive and credible leadership to junior colleagues, promoting and supporting the maintenance fo a cohesive multi-disciplinary team. Acts as a role model and assists members of the team with planning, priority setting and decision making. Responsible for provision and maintenance of accurate department based information systems including patient based processes, record keeping, care documentation and personnel data, in line with the Data Protection Act. Be self-directing, by identifying and monitoring the service provided and facilitate change in response to changes in health care needs and government strategies. To be aware of all the Trusts policies and procedures within the scope of responsibility. Ensure that effective reporting systems are maintained to ensure timely and accurate communication within the multi-disciplinary team. Effectively utilise resources required to meet the needs of the service, having an awareness of budgetary management. Assist with monitoring of activity levels, waiting times and orgainising scheduling within the Endoscopy Unit. Participate as a user in the Endoscopy User meetings. Education & Development Takes responsibility for planning, organising and delivering agreed training programmes related to speciality, teaching, supervising, mentoring and assessing all grades of staff. Responsible for the continuous development of staff and pre-registration students. Undertakes clinical supervision and act as a surpervisor of others. Undertakes mandatory training within required timescales; meets training requirements of the post. Undertakes relevant clinical and management/leadership courses to continuously develop specialist knowledge and skills of staff. Takes responsibility for interpreting and advising on the standards and requirements for training set by JAG and GRS. Takes responsibility for ensuring that all practice is established and implemented within policy and the service as governed by the Quality standards outlines by GRS and JAG requirements. Leads, creates and maintains a learning environment and promotes a learning culture for all staff and students promoting the use of E-learning to meet NHSLA and CQC standards whilst providing evidence of achievement. Supports learniers and ensures their understanding of the relevance of R&D to the provision of quality of care. Works within the NMC Scope of Professinal Practice, ensuring continuation of professional development through PREP and is aware of the principles of clinical governance and the responsibilities as a Registered Nurse for monitoring professional standards. Maintains close links with the Education and Training team. Maintain up to date knowledge of current research and best practice. Disseminate best research and practice to all the multi-disciplinary team. Provide education, specialist knowledge and training for staff/patients and carers. Takes responsibility for undertaking audits and benchmark, revise and review practices and standards of care BSG, GRS & JAG standards to ensure they are established and implemented within the service with the assistance of other senior staff members Research Has an understanding of relevant clinical research evidence affecting patients within own caseload. Is aware of specialty related research; facilitates the implementation of evidence based practice and relevant research findings. Responsible for the leading implementation of evidence based nursing care within their specialist area. Identifies relevant literature searches pertinent to patient care and supports staff to carry out these searches. Develops and implements a strategy for evaluation of health care interventions. Facilitates the opportunities and abilities of staff to investigate practice using the appropriate methodological approaches. Disseminates best practice through written publications and/or presentation both internally and externally, as appropriate. Assists with the R&D developmental plan for wards/departments in collaboration with the Multidisciplinary team. Understands the process of writing a research proposal and is aware of the Trust R&D Approval policy and the process for ethical approval. Initiates and coordinates clinical audit.
