You must be willing to work towards an MSc in Advanced Clinical Practice, including the Non-Medical Prescribing Module. Working across the movement disorder service the post is part time to cover the geographical area of Conwy., , Main duties of the job, , You will be responsible for the assessment, planning, implementation and evaluation of programmes of care. You will ensure that patients and their families are fully informed about their journey throughout the service and will liaise with other members of the community resource team to ensure the delivery of high quality care. The focus will be to support patients to safely remain in their own homes for longer, prevent hospital admissions, facilitate early discharge and reduce GP contacts.
