We are seeking to recruit two Transfer ofCare Hub System Discharge Leads who will lead a system-level Transfer of Care Hub (ToCH) for the Brighton & Hove 'place'. The successful post holders will lead the continuous development and improvement of the Transfer of Care Hub in line with evolving National Hospital Discharge guidance and good practice.
