Job description

Job Overview - continued The post holder will work collaboratively with other professionals and agencies to provide a high quality, comprehensive and dynamic specialist nursing service with the central aim of maintaining and improving the health and wellbeing of young adults with Learning Disabilities or autistic young people. They will promote and support the young people and their families to have a smooth and coordinated Transition from childrens health services to Adult health services This will include working closely with the young person and their family or care network, assessing and planning for dental support or physical health support, as required. You will be based within the adult Learning Disabilities team at BDCFT, however will need to work across a variety of different services, including dental teams, within the local hospitals and children's services, across the Bradford district. An understanding of the national drivers for Learning Disabilities services or autism is required as well as experience of supporting people with Learning Disabilities or autistic people to reduce health inequalities. An understanding of children's services and transition would be beneficial. Please read the attached Job description and person specification for further details about this role.
